WHAT WE DO

Customizable Medication Cycle

  • Fixed cycle - all routine oral solid medications for all patients arrive on the same day each cycle at preselected cycle lengths that make sense for your patient population.
  • Anniversary fill - all routine oral solid medications are delivered when they are next due to be filled at a prescribed frequency that makes sense for your program.
  • Refill on demand - program staff requests refills from pharmacy for their patients when medication is due, and pharmacy fulfills the request on-demand.

Medication Order Management

  • Physician’s orders - when appropriate for a given patient population or program, Physician’s Order Sheets (POSs) are generated by pharmacy and include all active medications on a patient’s profile to be signed by a prescriber to authorize renewal of all active medication orders. Life Tree maintains POS schedules and alerts staff when new POSs are due for patients. The POS system allows for all medications to have a consistent authorization lifecycle.
  • Refill acquisition process - Life Tree has systems in place to track refills remaining on active patient orders, and to proactively request refills from prescribers to prevent lapses in medication therapy and to ensure therapeutic continuity. Reports are provided to program staff periodically to show what medications are pending refill authorization.

Delivery Options

  • Life Tree employed drivers - for customers local to our pharmacy, Life Tree employs a team of drivers with years of professional experience to make multiple scheduled deliveries throughout the day.
  • Local courier service - Life Tree Pharmacy utilizes a local courier service to provide both scheduled deliveries and 24/7/365 on-demand stat delivery coverage.
  • FedEx - Life Tree Pharmacy utilizes various FedEx shipping services to ensure timely medication delivery to program sites outside of our local area as early as next day.
  • USPS - Life Tree patients receiving specialty medications are eligible to receive deliveries directly to their home via United States Postal Service.

Medical Records Maintenance

  • Customizable print MAR formatting - Life Tree employs a seasoned Medical Records department to maintain and generate patient Medication Administration Records that can be customized to fit the needs of your program and comply with applicable regulations.
  • EMR/EHR interfacing capabilities - FrameworkLTC, Life Tree’s primary pharmacy management software, has interfacing capabilities with an extensive list of commonly used eMARs and eHRs in the industry.
  • Life Tree portal access - once credentials are established by Life Tree IT, staff may access Life Tree’s secure portal from any browser to view and print patient medical records, admit/discharge patients, and place medication refills any time of day.

Consultant Pharmacist Services

A licensed consultant pharmacist provides expert clinical guidance to healthcare providers on the appropriate use of medications in a variety of practice settings. This role includes collaborating with members of the health care team by providing recommendations to ensure safe and effective drug therapy, as well as guidance on regulatory compliance, facility medication utilization trends and appropriate medication storage. Consultant Pharmacists will perform medication regimen reviews to access the appropriateness, safety, benefits, risks and cost-effectiveness of medication therapy on an individual patient’s health history. The consultant pharmacist may provide alternate treatment options, as well as medication management guidance including drug monitoring, administration, and storage.

Pacmed (Pillow packs)

  • Available in unit dose or multi-dose pouches
  • Each pouch contains the patient name, description of drug, and administration time (with many other custom fields available)
  • Easy pull and tear pouches
  • Provides for variable and shorter cycles

Blister Packs (punch/bingo cards)

  • Widely used and called by many names such as blister, punch cards and bingo cards
  • Available in unit dose
  • Comes in 30 count cards
  • Each card contains a prescription label, with all the information needed and has many other custom fields such as a resident photo

STAT/First Dose solution

  • Depending on the needs of your facility, we provide a variety of STAT and First dose solutions
  • Traditional emergency boxes can be stocked and switched out as often as necessary

Traditional Vials

The Caring Tree Program

Life Tree Specialty Pharmacy’s Patient Management Program utilizes an integrated, patient-centered, evidence based approach to identify and intervene with eligible individuals. Candidates for the Patient Management Program typically present with complex, chronic and/or multiple co-morbid conditions and demonstrate a lack of adherence to medication therapy, inappropriate medication use and/or adverse events.
